Update for patients of Dr Chandran

Sheppey-based GP Dr Srinivasan Subash Chandran retires from general practice on 31 January 2020.
The CCG has put in place long term arrangements for Dr Chandran’s registered patients beyond this date. Our priority is to ensure that they can continue to receive GP services and care, such as GP appointments, routine health checks and prescriptions when needed.
All patients registered with Dr Chandran’s practice are being informed individually by letter this week of arrangements for their future GP care beyond 31 January 2020.
NHS Swale CCG would like to thank Dr Chandran for his services as a local GP and wishes him well for the future in his retirement.
Patients are reminded that they can call NHS 111 or visit the Walk-In Centre at Sheppey Hospital, Minster, if they need to see a GP urgently, but their condition is not life-threatening.
