Future GP care for patients registered with Dr Chandran

Sheppey-based GP Dr Srinivasan Subash Chandran retired from general practice on 31 January 2020. All patients registered with his practice have now been informed individually by letter of arrangements for their future GP care beyond this date.

Dr Chandran’s practice was based at Sheerness Health Centre and operated a branch surgery at High Street, Queenborough.

The CCG held discussions with other local GPs to ensure all patients registered with Dr Chandran could continue to access GP care and services, such as GP appointments, repeat prescriptions and health checks, after 31 January 2020.

All patients currently registered with Dr Chandran have the right to approach any local GP practice of their choice and ask to be registered there if they live within the practice’s catchment boundary. However the CCG has made arrangements to ensure that patients can transfer to the GP practice closest to their home, if that is their wish.

A large proportion of Dr Chandran’s patients have the option to transfer automatically to Dr Simon Witts and Partners’ practice, which is also based at Sheerness Health Centre.

Dr Chandran’s premises at Sheerness Health Centre will remain operational, becoming part of Dr Witts’ practice. In response to feedback from discussions with the Queenborough surgery Patient Participation Group, the Queenborough branch surgery will remain open. This means many patients will be able to access GP services from the same premises in the same way.

Patients living to the east of Sheppey have the option to transfer to St George’s Medical Practice, which operates branch surgeries at Eastchurch, Warden Bay and Leysdown. Patients who are allocated St George’s can transfer automatically and do not need to register with the practice.

Patients living in the Minster area can choose to register as a new patient with Sheppey Healthy Living Centre, which operates a branch surgery at Sheppey Community Hospital, Minster.

Arrangements have been made for patients who live in Sittingbourne to transfer to either Iwade Health Centre, Iwade, or Memorial Medical Centre, Bell Road, depending on which is closest to their home.

A small number of patients who live outside of the NHS Swale CCG boundary are advised to register with a GP practice of their choice closer to home.
