Views wanted on changes planned for health and social care

 Our vision for the future is a place where people live long and healthy lives.

We want to help people stay healthy and when they are unwell, provide the best possible treatment in the right place, at the right time.

In Kent and Medway, we are working with other NHS organisations, Kent County Council and Medway Council to plan how we will transform health and social care services to meet the changing needs of local people.

It is the first time we have all worked together in this way and it gives us a unique opportunity to bring about positive and genuine improvement in health and social care delivery over the next five years.

We all want health and social care services that can meet our needs now and in the future.  However, we face new challenges that mean we need to change the way we work to improve care and get better value for the money we have available. Our Case for Change describes the challenges and explains why change is necessary.
